How to Register as a Patient

If you reside within the Practice Boudary, you will be able to register with the Practice.  Please click on the link below which will display the Practice Boundary.  If you wish to register and not make an appointment at the same time, it is usually better to attend in the afternoon as it is a little quieter.  You will be asked to complete some forms related to your personal details and previous doctor.  It will be useful if you could provide the following:

  • Your previous address and the name and address of your previous doctor
  • Your home telephone number and your mobile telephone number (if you have one)
  • Your NHS Number
  • If you have a passport or Driving Licence – please bring this with you.
  • If you have arrived from overseas, please provide the date on which you arrived in the country.

To complete the registration an appointment will be made for you to see the Health Care Assistant who will measure your blood pressure, take your height and weight and request that you provide a   sample of your urine.  She will also go through the personal medical history questionnaire with you.
