How to Register as a Patient
If you reside within the Practice Boudary, you will be able to register with the Practice. Please click on the link below which will display the Practice Boundary. If you wish to register and not make an appointment at the same time, it is usually better to attend in the afternoon as it is a little quieter. You will be asked to complete some forms related to your personal details and previous doctor. It will be useful if you could provide the following:
- Your previous address and the name and address of your previous doctor
- Your home telephone number and your mobile telephone number (if you have one)
- Your NHS Number
- If you have a passport or Driving Licence – please bring this with you.
- If you have arrived from overseas, please provide the date on which you arrived in the country.
To complete the registration an appointment will be made for you to see the Health Care Assistant who will measure your blood pressure, take your height and weight and request that you provide a sample of your urine. She will also go through the personal medical history questionnaire with you.

Your preference to see a particular doctor
We do of course realise that you may have a preference to see a particular doctor. When that doctor is working we will do all that we can to make an appointment for you to see your preferred doctor. However, there may be occasions when we are not able to oblige, and the reasons will be explained to you. To find out when your doctor is working, please visit the page called “Doctors Working Days” or click on the link to take you there automatically.

Car Park
The practice car park is located off Albion Street in Chipping Norton.
When the surgery car park is full, an alternate car park can be found on Albion street. Turn right out of the surgery car park and you will see the entrance to the public car park approximately 600 yards on the left hand side of the road.
From London Road approach (Oxford): Turn left into Albion street (directly after the War Memorial and before the garage). The surgery car park is directly after the first speed hump on the right hand side.
From Burford approach: As you pass the Chipping Norton School on the right hand side, keep going until you reach a mini-roundabout. Go three-quarters way round and take the exit into Albion Street. The surgery is located towards the other end of Albion Street. You will find the surgery car park on the left hand side directly after Portland Place.
On foot: From the centre of Chipping Norton, keeping on the right hand side of the road (shop side) walk east towards Chipping Norton War Memorial Hospital. A small incline to the surgery is located directly after “Harpers” hardware store. This approach is not suitable in frosty or icy weather and whilst the incline to the surgery is short, it is sufficiently steep as to not be suitable for people who have difficulty in walking.
