Temporary Residents

If you are registering as a temporary resident you will need to complete GMS3 Temporary Services Form for emergency treatment; immediately necessary treatment for periods up to 15 days and also from 15 days to 3 months.  Please ensure that you complete the part of the form that asks you for your usual doctors name and address.
